• Purchase of Comprehensive Liability Insurance no less than $300,000
• Completion of the CMS-588 (EFT) for the purpose of receiving electronic remits from Medicare
• Provider enrollment for the purpose of submitting claims electronically or completing a waiver if you qualify to submit claims on paper
